Advanced Nurse Practitioner

I can help by:

  • Managing a range of conditions: minor illness, musculoskeletal conditions, chronic disease, respiratory problems, skin conditions, wounds, and much more.
  • diagnosing, treating and referring on as necessary
  • performing physical examinations
 

Care Coordinators

We can help by:

  • Helping the doctors with triage to get the right care as early as possible
  • Signposting to the more appropriate services
  • Monitoring your health and care needs and responding to any changes
  • Supporting you to understand and manage your care
 

Clinical Pharmacists

We can help by:

  • Reviewing your medicines
  • Agreeing and making changes to your prescriptions
  • Advising about medicines and possible side effects.
 

Healthcare Assistants

We can help by:

  • Carrying out health checks and holistic assessments
  • Supporting you with long-term conditions (diabetes, hypertension, etc)
  • Promoting health and illness prevention
  • Administering vaccines and injections
 

Health and Wellbeing coach

I can help by:

  • Guiding lifestyle changes for long-term conditions: Type 2 diabetes, Pre-diabetes, Hypertension, High cholesterol, Obesity, and Low-level mental health. 
  • Focusing on nutrition, physical movement, sleep, stress management, and social connection.
  • Working together to develop personalized strategies, addressing motivation, barriers, strengths and creating an inspiring vision for your health goals.
